They are working on PoE 2 rn. Small poe 1 team can't fix this game. Move on and wait for PoE 2.
Try out predictive network mode in poe settings. If your stutter goes away its your connection.

"I'm experiencing massive performance issues in POE this league (3.23) and have tried loads of different things in order to mitigate the issues.
My rig:
CPU: Intel i9-13900K 3GHz
GPU: ASUS ROG RTX4090
RAM: 64 GByte
OS: Windows 11 Pro
HDD: WD_Black SN850X NVMe SSD
Internet: Connected via LAN, 250 MBit down / 50 up
Playing from Europe
Issues:
Playing high-density maps feels "clumsy" like walking through knee-high mud. I am experiencing massive input-lag when there's just a bit going on like during a Harbinger event, Altar-Mobs spawn, etc.
The FPS rate is stable at 120 FPS, so is the latency, though I've noticed that "Shader" is spiking.
Even when I'm in my hideout, moving around with Shield Charge feels laggy and clumsy.
The problem doesn't seem to be related to certain skills.
I am not experiencing any such issues in any other games - Cyberpunk on Ultra with highest settings is running like a charm."

I had exactly the same issue and only with PoE my system is similar to yours.
1. Go to Device manager then sound adapters and disable all but the main adapter (right click and disable)
2. Open Nvidia Control Panel then Manage 3D settings on the right now select the Program Settings it's next to Global Settings.
first select PoE.exe from the drop down menu scroll to Max Frame Rate and set to 58 FPS and click apply... later you can change it step by step to higher if it works.
On Steam launcher right click PoE click Properties in the Lunch options copy paste this:
-USEALLAVAILABLECORES
In game options, go to Video and make sure it's set to dirx 11 not 12. In sound options put channel count to Low. Run the game in full screen and make sure your resolution is set to your monitor native resolution.
This league didn't had any problems except the occasional internet lag mostly in towns.
