At the end game you'll run a lot of the same maps, but there's iteration to layouts and the monsters vary.
But yeah... it's an ARPG, there's repetition. The devs have made a lot of effort to give you some variety, but at their core, most ARPGs are about doing the same thing over and over. It's an FTP game, I'd suggest giving it a try to see if you like how it feels.

The core gameplay loop of an aRPG is grinding.
*However*, this game has several crafting mechanics that mitigate that. There's still a lot of grinding though - just for currencies (used to craft the enchantments you want on your items) instead of loot.
Basically if you just want to finish the main story, you wont need grinding at all, you can do that with items you find during the questing. If you want to play end game content, you gonna need to grind. Grinding allows you to craft/buy better gear, grinding is going to give you more levels, more skill gem levels,so yeah... PoE is all about grinding. It might not be your cup of tea, but people are enjoying it.
