For example, if you are level 85, the monsters must be level 77+ (i.e. Tier 10 map) to gain 100% XP from each monster that you kill. As the monster level decreases below level 77, you will gain less and less XP from each one. If you are level 89, then monsters would have to be level 81+, which is Tier 14 maps.
In any case, you are guaranteed to hit an XP penalty at some point. If you go above level 92, maps 8 levels below your hero will no longer exist. In other cases, the high tier maps will either be too expensive or dangerous to play all the time.

They never have.
I know they don't suffer the same currency penalties, all level 68+ enemies count as level 68 for the purpose of the currency drop penalties, so I'd imagine the EXP calculation isn't as strict as well.
If it was, the odds of anyone getting to level 100 is literally smaller than the odds of a mirror of Kaladria dropping, and Zod runes from Baal are less rare. :P
And I should point out that if the XP penalty from leveling was massive even in maps, Untainted Paradise wouldn't be giving people even a full level at 85. :) Which it can.
There is a major EXP penalty, but you don't need 10,000 Tier 15 maps to gain the last level from 99 to 100. :P
I've bought spots in that map many times during this Bestiary League, and the XP drops off noticeably as you approach 90. At level 90, my hero gained like 10% of a level (I was leveling some gems). A very small amount. When I was around level 80, I gained around 1-2 full levels. At level 65, you'll gain like 5 full hero levels.
Untainted Paradise is a great map for pushing into the mid-80s, and then it's not really worth it.
It would kind of make sense based on the fact that the next level requirements at level 90 become extremely large.
Since I've never run the map myself, I'm basing it on the times I've seen it be streamed or by information my friends who play this game have told me, that I probably misunderstood.
I'm still newish at this game, having only had one character hit 87. :P
You're right that it is a great map. However, the XP bonus is 300-400%, so eventually the penalty will start to override it.
You don't manually. The game is continuously saved automatically. Even if you ALT-F4 to quickly exit the game will have your character saved up to date.
