Gem level is the power level of the gem. Gem level is, in most cases, more important than quality.
an efficient way to have 20/20% gem is to level a gem to lv 20, then sell it with a gemcutter prism, you'll get the gem lv 1 but with 20% quality. Then you relevel the gem.

What most experienced peeps do is slot there main attack gem into secondary weapons that way you can have 7 attempts at making a level 21 gem, and you might get lucky and get even more that can sell for those juicy exalts.
