Level gem -> change gear -> not enough int, you can no longer use the gem (until new gear with int buff or you invest skill points
Some secondary skills used for utility can also be kept at level 1 to reduce mana cost, like flame dash, orb of storms, whirling blades etc.
Lots of reasons to not level gems.
Mana cost of attacks doesn't increase with level, so you might as well level them as high as you can. Lightning Warp needs to be leveled because it sucks otherwise. Flamedash maybe doesn't need to be leveled, I've never used it, don't like the charges.
That was from 2.1. I remembered the note, but I didn't remember that it was under the melee section of the notes, so while it doesn't say melee it was only for melee skills.
