If you don't got the hp to take big hits, you'll need to actively avoid them. Sidestepping. Not buffing him in the first two encounters. That sort of stuff.
Sidestepping can be difficult if you lock yourself in too much. Multistrike can be a really bad choice for fights where you need to sidestep.
More reliable than sidestepping is having a buffer to tank a hit, fortify, endurance charges, utility flasks. If you still get hit, recover.
3k life on level 76 seems very low, btw... comparing it to like tier 4-5 maps you'd be doing...
Your flasks are also ♥♥♥♥. Get one with armor and armor% (or any other defensive flask you like) and at least one with some kind of extra damage.
You are obviously not an experienced player, if you did not follow a build, make a new character and follow a build.
edit: depending on where you took that screenshot from, get more resists. You need 135% if you are in your hideout
Not even close to doing tier5. DPS barely scratched Izaro, but ok there was some strange 'gains endurance charges' thing going on i had not seen before. Still, it was too low to burn him with the 3 reset pillars provided.
Pretty much given up and rolling a homemade totem-templar (flame totem + searing bond + firestorm/srs) instead of the deadend that is my life&evasion BOWranger.
So far loving the templar (level 35), things die well and i can focus a lot more on position/movement since things like to go for the totems instead of my guy. I can even see a way to reach CI, though at unknown dps penalty. Hoping a switch to spell support totem will remedy that when the time comes. + hoping to farm a really good +fire damage talisman to boost dps.
Feel sooo confirmed in my original claim that evasion sucks, at least on ranger that has to stand still and can't be at or maintain range in some fights (or for example Izaro spams that green ranged attack or in some very crowded pitfights with no kite opportunity).
She destroys packs - good lord LA+LMP+CDS+projspeed+hatred, with high pierce, it is crowdmurder - but she lacks the punch on single target and specially the endurance, even when using a firearrow setup that has twice the tooltip dps than the LA crowdkill setup.
Evasion life builds actually do not suck, you just need to build them properly.

I followed a ice trap build shadow, I'm lvl77, 3.5k life with evasion. Merc izaro is very easy and a t10 map was a breeze, unfortunately I need more maps.
