The DLC shows up in the games properties though and I've deleted the whole game and re-downloaded it - still not there. So I'll say it again, it's a rip-off.

Delete everything in the two above directories. If you have old saved games you want to keep backup 238320 directory.
Restart Outlast. Every setting will be default btw.
Enjoy the DLC, it gets freaky and sick.
btw this method also removes the green tint from the camera night vision
