The game has traditionally killed all the boss/important characters on screen. All super psychos died in front of us. I noticed this pattern about bosses when I saw R. Trager's body in Whistlelower again. But there's no word on Frank Manera/the cannibal with buzzsaw. Last time we saw him he was yelling out of a window after we escaped from him in the lab. Then he vanished. Have I missed some optional piece, what happened to him?

You've said that correctly by yourself and no, You haven't missed anything from the storyline. After you escape from the hospital to the recreation area, Frank Manera will never be seen again in the Whistleblower DLC. Simply, He probably vanished and attempted on looking for another way to hunt you down or maybe had gone back to his lair to hunt down some other patients or the asylum's staff. He was probably alive because to be honest, He is the most dominantive variant in the hospital area and is physically strong enough to overpower security or the other psychopathic inmate patients. Other than that, Stronger variants like Chris Walker or Edward Gluskin are located in other areas far of reach from the hospital area so Frank Manera has nothing to worry about because he is the dominant one in his own place. It's like saying that Chris Walker is dominant and more recognizable with himself in the administration block or the prison block and sewers, Richard Trager is in the male ward and Edward Gluskin is residing in the vocational block - Each area in the asylum is invloving different dominant variant in there.

His death would be better and memorable on-screen in a glorifying way like the other majors. Such a waste.
