Additionally, I set the max FPS in the control panel to 142 (out of 144 for my monitor). Not sure if that helped anything, but it is running smooth now so there's that.

You're not looking at the V-sync drop down
Instead of trying hacks here and there, I decided to reset everything.
In the nvidia profile, I reset the profile for dxhr to default.
I also deleted the eidos entry in the registry (because I remembered that since my first launch of the game, I had changed my gfx card). When I started the game, the launcher appeared. I had forgotten this game even had a launcher. I left everything to the default values.
So the game's vsync option is on, using dx11. Since then I've maxed out the video settings(like soft shadows and ssao)
