Maybe it's a sneaky patch to preserve continuity? The character is quite popular, so we can't have her dying on us.
That, or the latest patch (Balance Update) broke the event
A) In Ridgehorn, if you let Ludin (and Bersi and Yrsa) go alone. In that case, Ludin and Bersi will die, while Yrsa will come back in Saga 2 (and it's a bit odd).
B) If you let her burn the woods and go away (there are two "fire" options if I remember correctly), in which case she get lost and you have to literally pull away the Prince, that will be hit so hard it will not try to go away alone, in Ridgehorn. Yrsa will eventually come back in Saga2, with a huge scar. (This if fine).
If you go for B, experience the dramatic Ludin reaction but she is clearly in rooster, it's a bug. If there is no way to have Ludin react to lost Yrsa in the burning woods, it probably a bug.

But like I was skipping through dialogue pretty fast because I knew how it went down, but then the scene didnt come up and we just skip to ridgehorn and I see the playermodel walking along
"Fight your way out as usual" = danger-1 & War
"Wait and see if they disperse" = danger+1 & day+1 & return to same options
"Create a forest fire as a distraction" = danger-4 & War
"Draw them into the woods and set them on fire" = varl-35 & fighters-8 & yrsadied & no combat
"Lure them into the woods without the fire" = varl-20 & yrsadied & no combat
"Lure them into the woods" = danger+1 & varl-10 & War
So, if you create a fire only as a distraction, Yrsa doesn't "die".
