It should be called Profile1.ctrls
Try just deleting that file, then launching the game!
I booted it up after a break, and all of my save files were gone. Initially, the problem was still there, but I switched to a different profile where it was working properly, and then switched back to the first one, and now it works. I made it to the first save and restarted, and it's still working properly. I don't know what the difference was, and I'm not sure why my saves were gone.
Edit 1: I was wrong about switching back to the first profile and having it work, I had swapped to the second one accidentily, which hadn't had the problem yet. The first profile still doesn't work properly.
Edit 2: Welp, I made it up to the motorcycle cutscene on this new game, and after the cutscene, the problem is back. Now my first and second profiles have the problem, but the rest do not. Resetting the progress on profile two does not fix this.
Note: Before the mouse reset worked, I clicked the numbers 1-4 to see if it would activate the action functions, and I didn't see any progress.
If that doesn't work, can you assign the attacks to non-face buttons?
My best friend was having the same issue (combat functions not working on controller but everything else working fine). What we did to fix it was mentioned above, somewhat. We went to his Transistor data file (see above for location), made a backup of all of the files, then deleted the .xml and the .xml.bak files from the folder. Upon relaunching the game, combat functions worked properly again (using an Xbox 360 controller)
We also made a Profile 2 in game, and swapped between that and his actual Profile, but I don't believe this had anything to do with solving the issue. Just tried it because it was mentioned above as well

Edit the profile.xml file in the folder listed above with any program, and find the line; delete it.
This worked a charm for me.
Still no dev fix for this - kinda meh
