I think that means they were both then reborn, as Saga and as Crow Mark II.

Zoe gave April hope again, gave her a purpose in life, and ultimately allowed her to use her powers again. She succeeded in that, which was more important.
5) Yes. Zoe was one of several babies genetically modified by her mother Helena Chang (including Faith from 'Dreamfall' and Hanna in this game), to have the dreaming powers. Her mother released her from the lab as an infant, at her father's request, because Zoe apparently had no powers.
However, in Dreamfall Zoe was able to fall asleep in Stark and create a new body for herself in Arcadia, which was able to interact with people and affect reality there as if she were actually present. Seeing this, her mother kidnapped her at the end of Dreamfall and put her in a coma.
During the first four parts of Chapters, Zoe is actually still in a coma, but she's dreaming she's awake and moving around Stark, and has actually created a dream-body that can affect reality there. When she travels to Arcadia, she's still dreaming that as well. In chapter five she finally wakes up for real.
Her mother wanted to use Zoe's powers (while keeping Zoe herself in a coma) to change reality, yes.
