That doesn't say anything about:
1. The crime being witnessed (your target can be seen dying without you being spotted)
2. A body being found (unconscious or dead)
3. whether you killed or allowed to die a non target
4. Whether you left any recordings of crimes or dead bodies
You have 4 other requirements beyond "not being spotted" in order to get a silent assassin rating. It requires all 5 conditions to be met and a 5 star rating
Without any further information that's all anyone can tell you. You've not mentioned what mission you were on, what else you might have done wrong or even what the game listed you as failing at.
If you want someone to diagnose the issue, try recording your run and post a video or at least provide a screenshot of the detailed score page along with a better description of the problem
With the complete lack of information you've posted no one can tell you whether you failed one of the other 4 conditions that you haven't commented on or if the game simply bugged out with a glitch
Apparently not only can you not get spotted, you can't be spotted enough for them to even realize something's there. If they think they see something and come to investigate then according to the game you've been spotted, even if when you knock them out it doesn't say unconscious witness.
I had to redo the whole thing, but I got suit only silent assassin in the end.

Silent Assassin as a rating has nothing to do with what suit or disguise you wear at any time
In fact the way you word your question implies being allowed to change out of the suit and then back in to leave: There is no challenge or rating in the game that requires or even allows this.
The only Suit related challenges clearly state "Suit Only" and those require you to start in a suit and not switch to a disguise at all, so you will automatically leave while in the Suit as well for completion. However, "Suit Only" is not Silent Assassin, these are two distinct requirements.
If someone were to discuss SASO this stands for "Silent Assassin Suit Only ", it is a Classic Challenge on each of the missions and a regularly used condition in contracts mode, however the two parts of it "Silent Assassin" and "Suit Only" are still independently earned as one has nothing directly to do with the other as a rating. (Suit Only isn't even used as part of your score and only exists as a challenge)
Just to confuse the conversation further: contracts mode additionally has the optional condition of "do not change outfits" which let's you start in any suit or disguise but does not allow you to change at any time during the contract (or you fail the condition). But this does not even require Suit Only, it let's you remain in whatever disguise you chose from the start.
I know of no rating, challenge or contract condition that tracks or allows anything related to 'switching back to your suit before you leave', that's a purely personal preference that some gamers have created out of OCD habits
If you switched out of your suit, you've lost Suit Only. Switching back does nothing for you. And Silent Assassin rating (by itself) doesn't care what you are wearing: ever
