If it was intentional, they would've listed it on their Release Notes of the GOTY update so I don't think so.
And it might be the point is actually to sneak weapons past the guards and into that wing. (since there's even a feat for that)
Kind of defeats the purpose if you're going for the "(not) malpractice" challenge. Need to have the head surgeon conscious for that one.
I had a hard time trying to get one and gave up as The Director with an all access pass to every single door in the facility. Instead I knocked out a bunch of people in a side room, quitely murdered them. Switched to a guard outfit, commited a mass shooting as a guard so I could gain access to the operation area computer to upload some data, after the carnage was done, I hid in closet to wait for things to settle down, when the coast was clear I then switched back to The Director and was able to proceed from there...
