Monk from the Sea boss is missing from my journal

Monk from the Sea boss is missing from my journal but I have his spirit collected and use his transformation.. there was also a bug after beating him, that I was still stuck in the boss area with the invisible walls and had to reload. Can you please check and let me know if this can be fixed ?

I was able to get the missing journal item for the Monk in NG+.
