How to unlock Revengeance mode?

There is something confusing me. I played through the game on hard and on very hard. But there is no Revengeance mode, so i put in the Konami Code, it did not unlock the Revengeance mode.

How to unlock it? What is the normal way to unlock it without the Konami Code?

Wrong code? It's the correct way...

Unlock Revengeance mode:
On the title screen, enter the Konami code (Up, up, down, down, left, right, left, right, circle, x). Revengeance mode will be made available as a difficulty setting.

It's weird that the menu sound "Metal Gear Rising: Revengeance" was heard, but there was no difference in the available difficulties.

Anyway, i am half way through Very Hard again, let's see if this time something changes.
I vaguely remember that i haven't played Very Hard in one run at the first time, but resetted some chapters inbetween for Achievements. That was probably the cause.
