Pressing on "continue" upon completing the game will start the game from R-00 on the difficulty level that immediately follows the one you just completed it on. So if you did it on Normal, the game will restart on Hard, and so on until you hit Revengeance difficulty.
I imagine that's because "continue" always resumes from your last checkpoint, and since the game saves after completing the final chapter, there is "no place" to resume from. So it starts from the beginning again. You can always override this by starting any individual chapter you like from the "Chapter" section.
Avoid the "New Game" option like the plague and use the "Chapter" option instead; that'll keep your save file safe, which means your upgrades, collectibles, etc etc. will stay. For the purposes of starting a new run, picking File R-00 from the chapter select screen will always be treated by the game as that.
If you want to go even further than that, you can get all titles ingame. You don't need all titles to get all achievements. (Titles literally don't do anything though so idk. I only got all achievements and was satisfied)
An irritating title is the one where you have to complete the game over 10 hours, with 10 deaths or more and used 20 health suppliers or more.
I beat the game in 3 hours.
