Don't try to rotate your cut to line it up. Aim your cut horizontally/vertically (depending on enemy position) and use WASD to move it to the red box until you hear the "ping" confirmation.
You can easily aim your slice direction by swiping your mouse in the same direction you want your sword to slice (swipe left/right to cut horizontally, up/down for vertical)
You can get easy practice with regular cyborgs. A horizontal slash with a tiny WASD adjustment can zandatsu them near instantly while they're standing. Use the slide attack to practice vertical cuts.
When you get to precise multi-box blade modes, do your best to swipe your mouse in the general way it wants you to cut, and leave all the movement to WASD.
Also yes, the red square isn't perfectly precise. I've had times where i've cut outside the square and gotten a Zandatsu, and times where it told me I was lined up and missed it still.
It's always in the center of the enemy, though, so a straight cut vertically down the center always gets it (except with striders, it's practically RNG with those things).
