Report a translation problem
After playing with default advanced options on several games, I decided to play around with some advanced options. I maxed hunger time/nests and increased chance of death to 2.5
I also installed a mod that tweaks the rest of the options aside from global damage multiplier:

So far with all of these settings combined, my game experience has significantly increased in terms of difficulty, if that's what you're looking for. I found the default settings to be way too easy, especially after setting up a base.
Balance settings should be chosen based upon what appears are the best choices for how the game is currently balanced... That's the best choice for "gameplay" which is the reality that you're in whenever you're playing a game.
After that, adjust for "personal preference" instead of "Reality."
I leave everything at "1" because that's what it appears the devs have "balanced" around. The one setting I do change is the Hunger setting and that's to 1.5. Why? Because food is already a non-issue in terms of access and I have better things to do based in a Town, and not my own Base, than to run back and forth all day from The Hub to Squin hauling food for 30 characters... That's "my" concession to the "1" rule for my own gameplay experience. In gameplay terms, the concern over food is either a Day 1 Noob Problem or has been impacted by the player's gamestart choice. That's my excuse. :)
1. How much time should it take for a human to starve when they're sitting there doing nothing? Adjust hunger rate so that it matches that.
2. How quickly do humans bleed out after sustaining injury that would incapacitate them? Adjust chance of death to match that.
3. How long would it take for a person to be incapacitated in real life? Adjust global damage multiplier around that.
4. How much housing is there in the world? Adjust population so that it makes sense.
5. How long would it take to craft a certain item? Adjust production speed so that most item craft times make sense.
6. How many bandit camps, animal nests, etc. would there be in a world with this much surface area, taking into account distance between towns, access to food/water, etc? Adjust number of nests multiplier so that it makes sense.
Well, I'd have to agree with what Morkonan said in that case, that realistic is hard to define in a game. Instead, you should focus more on your personal preference. What others may deem as more realistic for them may not be the same as in another person's opinion. I tend to play a lot games where the focus is in survival elements such as food/water/resource management so the default Kenshi settings, especially regarding food, was way too low for my preferred playstyle.
