Spawned items should stay as far as I know, at least until their respawn timer (usually 24-48 hours), if not longer.

Dropped items however will despawn instantly after leaving the loaded cell. But if they are loaded the whole time, they will stay forever.

What do you mean by "leaving the loaded cell"? The thing is that I have a dropped item stuck under an unaccessible terrain (after I disarmed a wall close to it), and my characters will try to pick it up and get stuck next to it, because its too far for them to reach the object. That's why I would like it to disappear, and I don't know if its enough to just take all my characters far from this place for this.

The map is splitt in invisible chunks called cells that - for performance reasons - are only loaded, if the player is present in this cell.

Simply move fare away and that area gets unloaded.

It's easier to tell with cities, because once you get far enough away they lose a lot of detail. At about that point they've been unloaded and walking back up to them will initiate any world state changes (if you've killed an important NPC in that area or whatever).

This mod shows the cells on the map. The game keeps a 3x3 area loaded around your camera, plus any cells your characters are in, and any cells needed to fully load a town in case part of it overlaps the cell borders and would normally be partially unloaded.

If you don't want stuff to despawn there's a mod for that. The mod creator also has other stand alone versions for only backpacks, or other specific things, and that applies to "everything".

I have never proven this in Kenshi but many other games will become unstable if the player uses the world like a garbage can b/c of save file bloat. Kenshi save files are already large and why devs do not allow cloud saves, so caution recommended.

Use containers to get rid of unwanted items, they are only a line in an inventory record then.
