It's just that you have the ability to potentially wake up and bandage yourself all the way down to -99 at max toughness.
Also make sure to turn on stealth for any character you want to stay down once they drop. They play dead and heal themselves and can move around slowly in a crawl to heal other party members (their stealth score is doubled while crawling).
So you can KO even before you reach health KO point by getting a lot of 5 damage? Problem is that a good armor blocks damage so you can't get a single hit large damage and get KO before you reach limit.
Yes but if your limit before KO is -101 and death happens after you reach -100 on critical parts how you gonna protect your guys for staying till the end?
but if you lose a someone, well thats just the life in the game.
cant win them all.

when they drop unconscious.
any hits below the negative mark will instantly knock them out. and the portrait turns white of the affected character. then you get a count down timer of when they regain consciousness.
thats your warning they are in danger.
The only difference is that they can potentially wake up in time to save themselves/others instead of everyone just dying from their wounds because they're in a recover coma and can't bandage their wounds to stop degeneration, which never stops naturally, it only gets worse if left untreated (though higher toughness does slow the deneration down, buying you more time to wake up or get bandaged).
The lower the critical parts get, the easier it becomes to get staggered and even a few points of damage can KO you for 15-20 seconds at those levels.
my solo guy is 95+ in everything with 100 toughness and he still gets blacked out in a few hits once he drops below -50. But since his KO point is -99, and his wound deneration so slow, as long as it's not an enemy that eats you, or some don't come by before he recovers, he'll eventually wake up and can heal himself and can do so all the way down to the point of death.
But sometimes you die in Kenshi. That's simply a risk you take by getting involved with battles. Sometimes you die instantly from a large blow. Other times you're slowly bleed to death in a herioc last stand against insurmountable odds. other times you get KO'd and die from infected wounds due to a stray crossbow bolt from a battle you weren't even involved in that you were just passing by.
unless you're constantly throwing your people into battles without giving them time to recover, you never really have to worry about dying if you have high toughness. And if you are getting pushed into constant battles, you're likely in an area too dangerous for what your group is currently capable of.
But being already beaten half to death and then fighting a big battle, you are probably gonna die or at least lose some people, since you're at the point where nearly every blow will either KO or Kill you.
But a less amount of toughness would've already died by then before you even got to that point.
You die if you go over the amount of your body part. if its in your head, or stomach or chest.
other parts they get chopped off. but you can still die if its a bleed out.
Its possible to go over. i recently had one of my people get three bolts to the stomach form my own people. put her at -101. she went into recovery coma. only because her body part was higher then 100. other wise she would have died.
Then you either reload to get them back or go with the flow and go recruit someone new to replace them.
