I noticed the placement of the stables causes some problems.
If it's right next to a wall, then the horses get stuck in the wall when they try to leave. They become visible when you tear the wall down but they don't go to the knight. You can't delete them so they just stand there. For that matter, I don't think you can dismiss ANY units
If you face the stable towards a wall, the horse gets stuck between the stable and the wall and won't move. As you said, the knight shows that he is mounted even though the horse never got there.
When I build stables now, I allow a path so the horses can leave.
Spacing can be a problem with other buildings. I placed a blacksmith too close to something and when I checked on production, it said a worker couldn't get to the building.
