1. Check your inventory ASAP because you should have something more interesting by this point.
2. Don't forget to upgrade your armor and your weapon
3. Don't hesitate to use Nail gun (it does helluva damage) and grenades (if you have them). Craft some nail gun ammo from time to time.
4. Try to attack from behind and if outnumered run in circles and wait for them to make missed attack - this should open a short window for one-two hits.
At this point in time would start practicing using the sand throwing ability, I use this every fight, there's is almost always 1-2 in groups that don't have any head gear. Also set your follower on defense, while not very strong, Innocence will buy you alot of time distracting several goons.
The idea is to incapacitate a good number while working on 1-2 at a time, almost every encounter you will have atleast 2 goons on you that will sync their attacks to disrupt you.
Be patient, wait for a counter and while he is momentarily staggered, tee off on the other guy before he can attack. Throw in some guard breaking attacks also so they don't adapt to your chaining.
Repeat this while continually blinding people whenever you are able.
Remember that blinded opponents will still swing while stationary and CAN damage his allies, so position them accordingly.
first room: I sneaked to the left and smacked the first mole in stealth, then just went on attacking and killing them while innocence hold off 2-3 moles, after my 2 were dead innocence "died" causing them to storm at me, so I basically kited them(hit, run, hit, run and so forth).
Second room I sneaked to the corpse looted the grenade, sneaked behind the big guy and placed a mine, then smacked him and jumped back and generally just focused on killing the big guy(and threw a greneade when they were all clustered together at some point) first then the small ones(also make sure not to walk too far into the room or you'll get 2-3 extra guys).
After those 2 rooms it's generally not too hard.
Kill 1 by 1 while Inocence is drawing the majority of attention away from you.
But you will want to do all the sidequest first, they'll give you gears and useful stuffs. Or just buy better gears from the traders.
I finished the game on medium, and only die 3 or 4 times. You'll live if you do not shy from spending your serums and items.
The first room is quite easy as you can kill them all one by one, sneak and attack from behind when they're alone and PREVENT THEM FROM ALETING THEIR FRIENDS by attacking them when they try to make this loud shout that wakes up and alerts every mole in the room.
The second room, well it was nealy impossible in extreme mode so, i went back, finished the quest that gives you the trap bomb plan, made 10 of those and planted 10 trap bombs at the same spot, ran in the room making ninja rolls to pop/aggro them all, made a nice mole ball and lured them all to the traps; BOOM. Next thing i know i got one injured suvivor to finish.
It does get a bit more interesting after you get technomancy, raise your stealth skills a bit, enhance dodging and parrying, etc. You'll find yourself rolling a lot less and actually engaging a lot more.
Protip - enhance rolling via a skill and instead of moving around by running in combat, just roll to the new position. Much quicker. Seems unintuitive since running on your feet would be quicker in RL, but that's how the game does it.
