Some perks, like Commando as mentioned, can see him at all times while he is invisible. Other perks can contribute greatly and you can get him before he heals easily, most notably Sharpshooter. A few of Sharpshooter's freeze grenades can freeze the Patriarch right in his tracks when he tries to run. However, you need to be quick, and you or your team if you're playing online need to take advantage of that moment as much as possible.
Otherwise if your team can't dish out enough damage, you just gotta drain him of syringes. You will know when he has run out of syringes when his healthbar turns red. It changes colors from Green-->Yellow-->Orange-->Red. Hans (when you fight him) will do this as well and has healing capabilities, but he has to catch a player first. He puts up a shield that you can break to cancel his healing ability, but he will still have to go through the same health phases at the Patriarch until his bar is red before he can finally be killed.
