Achievements
Achievements are not unlocking. 'The Last Unexplored Path' and 'you're not supposed to be here!' are broken.

When I was brought back to level select EPs 1-3 were unlocked again (for some reason) and the final boss was inaccessible.

I've since edited the topic but it still applies. I would have The Last Unexplored Path if the game registered me as completing EP 4.

By starting a new game and completing EP 4 I've unlocked 'The Last Unexplored Path'.
Note: It was a straight shot with no quick saves.

Are you using New Game or Level select?
Level Select doesn't unlock achievements.

In my original play through I only loaded from saved games. I also heavily abused quick saves but some people believe that to cause achievements to not unlock.

Then I have no idea what happened the first time because not only that but I couldn't face the final boss despite having all four runes.

That happened to me once with vkQuake! I ended up playing through the first episodes again, for fun, and unlocking the final boss :D
