I recommend to try this mod, Manly-ass doom. It's not what exactly you are looking for but you'll be satisfied with it. It features Perma-death, new difficulties, and random level generation.
If you play this mod in random level mode, it offers everything you want : no puzzles, no deep exploration, just you and bunch of enemies cause randomly generated levels are not very complicated.
I was also looking for arena/survival maps like you, But all of them I have found were kinda dull, and I started to realize that Doom doesn't work well with wave-based survival. After some waves the map is filled with lots of corpses which is kinda annoying (Especially mancubus and Cacodemon corpses), and finding 1 or 2 remained enemies in the entire map is tedious. These problems can be solved by playing new random levels everytime you die with Mandoom.
I have already played Mandoom for many hours but I'm still not bored. You have only one life and you can't save except when exiting the game, So it's truely a survival.
I play it with vanilla doom 2 but you can also play it with other mods like brutal doom.
