The germans did indeed have a rifle grenade launcher for their Kar. 98 called the Scheissbecher. They also had the PzB.39 anti-tank grenade rifle, which could be used for HE grenades as well. Finally, they had the Sturmpistole, which was a flaregun modified to fire offensive grenades of all sorts.
The Soviets had a shoot-through Anti-infantry grenade that didn't require special blanks called the Djakonov rifle grenade, which you can see a diagram of here (unfortunately still in Russian): http://www.lexpev.nl/images/djakonovgrenadecommercial.jpg. This would be good because the Russians didn't need special cartridges and could supposedly keep shooting right after launching the grenade.
They also had an anti-tank grenade called the VPGS-41, which you can see an english-translated diagram here: http://www.lexpev.nl/images/grenadevpgsenglishforlex1.jpg. The problem with this one was the intense wear and tear the guiding rod (inserted WAAAY into the barrel of the gun) dealt on the barrel of the rifle, and it also required blank cartridges.
Probably the best way to handle the damage that these rifle grenades did to the guns they were used on could be simulated by requiring a special barrel modification that reduced accuracy and costed more to repair than the other barrel options. Also, you would have to carry the additional grenades seperately from the gun.
The best part about this would be that this could help pave the way for bayonet attatchment to guns, since you're modifying the gun itself mid-combat.
