The map which the issue occurred was the same both times: the one which he makes his first appearance during the campaign, with the sewers and the randomly placed rift portal.
First time was the initial encounter, second time was from a negative modifier through map select.
Maybe he spawns at a spot where he shouldn't and he's automatically killed...?
Just now, group of three. Harlow, Vaan, Karlos. Our modifiers at the time were:
-Earth Elementals
-Kobold Sappers
-No Selling Traps
-Infected Kobolds
One of our party said they reached Ghostfang solo and he spawned alive and well, but each and every time our group got to him as a group, he didn't spawn properly.

the worst part is we still lost cause we had dynamite archers and a single dynamite wiped out an entire 3x3 grid of barricades sigh
Had 0/100.000 Health right after his sequence.
After a while we won.
Was the first time we reached him in the 5 Mission run, 2 players, Kalos and Harlow.
Map was the Backalleys, which was a new one for us, so I think its his "native" map.
We cleared out the orcs and the round just ended and we won.
Never actually fought the boss, he spawned in with 0 HP.
This was on the sewer level as well, first time we encountered him.

For us, he instantly lost all his remaining health because he walked into the ward I placed. Perhaps previously he could spawn on or immediately waltz into objects with similar properties as this. Maybe this can be abused on all bosses?
