Can't find all places to liberate

So, I'm having problems with liberating the provinces, for example Lavanda. I already liberated the harbor there and the two small towns but the game only told me I liberated 3/4 places. But I can't find a 4th place in that area! Also I'm having the problem with other areas too. Am I just too blind to find all places or is that some kind of bug or something?

i've struggled a little to find those places too
some areas to liberate are hideouts, open your map and search for grey dots/places on the map, is likely that those places are the hideout. (a checkpoint, outpost, etc)

They're all there. I didn't like how some places are tiny specs, either. It's best to just jump into a chopper and fly around in a circle, paying close attention for any of those signature red destructables, and those "being watched" alerts.

4th place is a small military base north of Rondella

Wait till night, fly at anything with lights that are more than a single house, also destructible areas have red lights on them.

Its a small base in middle of 2 towns, Albeto pero and Rondella close to the sea
