In any case, update all the drivers you have.
Check first, if your xbox controller really works correctly, by testing it:
Win-Key + R
Type in "joy.cpl"
Click on Properties and see if all the Analog Sticks work correctly.
I never had your issue with THIS game, but I had the same thing with old DOS games and one Windows era space sim.

First, check if the "Steam Controller API" is not interfering with your device. Steam > Settings > Controller. In addition go to the Fallout Game Library Page and disable Steam Controller Support in the "Properties" section.

If unplugging and/or plugging into a different USB jack does not work, you can do two things:
Goto Windows Control Panel > Hardware and Sound > Device Manager > Human Interface Devices and uninstall all "USB Input Device" entries.
Don't worry. All the connected USB peripherals (keyboard, mouse, etc) will be detected and the drivers newly installed.
This is just a common way to allow Windows OS to 'clean up', if a user has plugged in USB peripherals into different USB slots multiple times, which can be confusing to the registry.
... though, I doubt that this is here the issue or will solve it.
What you probably need is a game controller driver.

This might sound strange, but this can help.
Use the x360ce driver wrapper (googlebing). It is a tool that does what Steam now also does: allows you to connect any controller, but pretending - in this case - it is a 360 controller.
It also works when using a 360 controller.
This might solve the issue, by just adding a Dinput.dll in your Fallout3.exe directory, bypassing the regular input libraries.
I tried everything here except the x360ce driver wrapper and it still is doing it. I thought it was a driver too but everything is up to date. The controller does fine on all my other games steam and not steam. So I will try that wrapper thing next, Thank you for the help if you think of anything else that might work I appreciate the help.
