If you don't want to install a ton of mods to fix it the best way I could find was to edit these 2 lines in your Documents\My Games\Fallout3\Fallout.ini
iFPSClamp=120
This speeds up the physics engine to match your fps which also increases your fps when it's not set to 0. However it also effects the game speed depending on your framerate, you should set this to be just under your average fps, not your fps cap. I have 165hz monitor and found 120 worked best for me.
fMaxTime=0.0083
This fixes the game running too fast when you have too high of an fps, again this should match your average fps not your fps cap. Use the table below to find the right value that works for you.
fMaxTime=0.0333 for 30fps
fMaxTime=0.0166 for 60fps
fMaxTime=0.0133 for 75fps
fMaxTime=0.0111 for 90fps
fMaxTime=0.0083 for 120fps
fMaxTime=0.0069 for 144fps
fMaxTime=0.0042 for 240fps
Changing these values made my game run at 155+ fps but it slows down to 120fps in combat which is why I went for the 120fps values.
The combat bug has to do with the health bar animations which can only be fixed with the FOSE or capping your fps to 60 in your graphics card control panel. I decided to just dial in my physics settings to stay above 120fps.
Thanks, it didn't work with the folder the main post gave, but that did it.
