How does the mysterious stranger work?

Doing Mothership Zeta right now and I clicked the mysterious stranger dialogue and nothing happened, even after the fist fight.

How does this work exactly? I know some guy shows up to help you but not sure the mechanics of it.

Every now and then (I think the frequency is determined by how much luck you have) a dude pops up out of nowhere once you are attacking in VATS and one shots your enemy.

It feels kinda cheap but it's awesome when he gets you out of a very tight spot.

If I had my time again (and I might in 4 not sure if the perk is included) I will probably not go that way because I like my games HARD and knowing what I know now that he is essentially a get out of jail free card I would prefer to go it alone...
