if you are on AMD GPU i had to roll back driver to 23.12.1 to get the game to work
i dont know what the deal is with AMD and Bethesda but their drivers broke not just Fallout 3 but Skyrim AND Fallout 4! I had to role back my drivers to back then to get them to work on top of doing fresh installs of all 3 games. complete purge before install as well.

Glad to hear that at least you found a workaround for whatever is going on. Weird though, the games are working fine for me and I am using the current driver and software. I'm using an RX6700xt if that matters.

AMD are working on a fix:
"Fallout 3 and Fallout: New Vegas may fail to launch.[Hotfix pending verification]"
in the latest patch notes https://www.amd.com/en/resources/support-articles/release-notes/RN-RAD-WIN-24-5-1.html

Wow so it's been like this for almost 6 months and they're just doing something about it. What a joke. BTW did you update your drivers?

Two old games are hardly a priority for AMD, unless Microsoft leans on them hardly going to rush to fix it were they. Only got mentioned in the patch notes after the boost in player numbers
