For a non-mod solution on Win11 that I found to work: Go to settings, go to DISPLAY settings, go to Graphics, then set custom graphics settings both for the Fallout 3 Launcher and the Main EXE file (At least, I did it with both as per what I originally read and it worked out well enough). Set the both of them to "High Performance" as opposed to letting Windows decide.

I've seen this this solution before but I didn't get what settings for the launcher etc?

It's very simple.
Open your settings "app". Should be represented by a little picture of a gear?

Next, on a list there should be a button that says "display" with a little picture of a monitor/laptop and the words "monitors, brightness, night light, display, profile" (Note: for me it immediately pops up when in a smaller window; at fullscreen it is behind the "System" button from a list on the far left of the screen)

After clicking that, you should be met with a different list. Scroll down to the one that says "Graphics" and click it. Now, you should see a list of other apps (including games). Click on the one that says Fallout3.exe (NOTE: If not present on the list, you can browse through your files for it using the appropriately named button a little bit above the list to add it).

Click the "Options" button. You should see a smaller menu labeled "Graphics Preference"

Click the bubble next to "High Performance" and then hit the "save" button. No other changes need to be made.

Repeat this process with the launcher file FalloutLauncherSteam.exe. Add said file to that list of apps as necessary.

Congratulations! You *should* now be able to play the game without messing with any mods or modifying game files.

And it still won't run. The launcher does, but the actual game ctd:s instantly.

Edit: It was a d3d9 issue. This https://www.nexusmods.com/fallout3/mods/17209 (Intel HD Graphics Bypass Package - from Nexus ) Fixed it. And I don't even have Intel integrated Graphics...

That's certainly odd, although somebody else responded in another thread that the fix no longer works--possibly as a byproduct of Bethesda pushing out one of their weird it's-not-broken-enough patches. I was playing the game fine up to the tail-end of October last year.

Either I just had lucky timing and did my first and only playthrough prior to Bethesda bungling up QA as usual or my system just exists in the hardware version of a Goldilocks Zone.

Later on, I found that it was the wrong fix.. I maybe it did work, but caused problems later. I just undid the proxy d3d settings from ENB inis, reverting to normal enb behavior. I may have then done this same thing. Either way, it works now. It just works.
