Yeah you do remember certain quest sequences, right? No matter how many years pass, you're like I know this!
Like if you don't find Riley nor a certain member of her squad injured and out of ammo in the hotel when you figure out you can't reach the upper floors without backtracking and finding the hospital to cross on the fallen broadcast tower... last playthough I went into the hospital first, ended up on the hotel roof, and it does a pretty good job of picking up mid-mission... the thing plays out as though you did find both, except there's no explanation of why you have "here I brought you ammo" choices when you talk to them...
The devs knew people would accidentally trigger quests when developing an open world playstyle, the accommodated us pretty well actually. There's very few hard conflict/blocked mission paths, it is impressive.
I remember one time the Annandale cannibals attacked me on sight, and I never went into the shed or their basements. Never did figure out why they were hostile to me, but it's bugged me for years, everytime I think about FO3. So I get the not wanting to glitch a quest thing.
