What exactly do you do to 'use' it? I have not seen any prompt or button to 'use' something while the pipboy is open.
Even the wiki says 'Reading a skill book permanently raises the corresponding skill by one point.....' without any explanation as to how skill books are read.

You might have been thinking of the Bobbleheads which are are automatic stats once picked up. These changed going into 76 to be a use item that grants bonuses for a set time similar to the magazines in New Vegas.
What happens if you already have a skill at 100 and 'use' a skill book that adds to that skill? Is that book wasted?

But you would already be at 100 so what does it matter?
you can’t raise any skills above 100. using the book just consumes it if a skill is already at 100.
