To solve it you must go to "My documents - My Games - Fallout 3". (Remember, don't go to the steam folder). There you will find a file called Fallout.ini (or "FALLOUT"), double click and look for the "Audio" section, in it you must change 2 numbers (it is a text document, you click on the place you want to write and write the number that you have to put as if it were a Word document, then click on the X to close and that's it)
Find a line called "fDialogMinDistance =" and change the number to 25.00. Then find another line at the end of the "Audio" section called "fFilterDistortionGain =" and change the number, put -7.5.
I also played the Xbox 360 version and I could also see that there was a bug in my steam version. The audio notes and the people wearing helmets were heard loudly low and it is impossible to hear them.
If when you fix it you can, for example, hear the audio notes well but not people with powerarmor google something like "Fallout 3 voice bug" or "Fallout 3 sound bug".
