John Rambo'd achievement help

So, I started a new game to get this and one other achievement, I've tried places like clinic area, mall, bank, library, front street of dude's house, area in front lucky ganesh and the police station. Pedestrian number is set to 30. Only things dude says while shooting is "Only my weapon understands me." and "Today's the first day of the end of your lives." By the way, does wearing the police uniform matter?
