I just unlocked this. The thing that can cause this achievement to bug is if you are playing on pretty much any difficulty higher than Liebermode, and here's the reason why: In the opening cutscene that shows how the Vend-A-Cure XJ2 models work, when they start rebelling, the scientists will whip out guns to use against them and WILL kill at least one of them before the cutscene ends on difficulties where they get weapons (I was playing on Hestonworld and saw this firsthand; one time they even killed ALL FOUR OF THEM). On Liebermode, the scientists will simply try (in vain) to run away from them when they attack.
In order to unlock the achievement, all 4 of the XJ2 units in the opening cutscene must survive the cutscene long enough for you to be able to get close enough to them to "deactivate" them. If even one of them is killed prior (or if you kill one other than urinating on them), the achievement won't unlock and you'll have to start from scratch.
Also feel free to speedrun to this day by playing on Enhanced Mode and using Shovel Flight to get there faster; I just unlocked it on Enhanced Mode and had no issues; just be sure to play on Liebermode to eliminate the chance of any of the 4 XJ2 units in the opening cutscene from getting killed by the scientists before you get the chance to "kill" them the proper way.
Anything higher than Manic will ruin it, though.
The cutscene will be weird because are no interactions with NPC (just the robots talking) but it's possible to accomplish. In fact, you can use this kind of exploit to advance in the game or avoid some encounters in several situations.
