"Low-res" (blurred) and "stretched" are two different things.

Hey there! A little bit late to the party but I tried your fix and it worked great. I played through half of the game and no problem was found.
I took the liberty of adding your fix to Quake 4's page on pcgamingwiki.com. I hope you don't mind (I credited your Steam username).
I play the game in Italian language, maybe it works only for English or some .pk4 overwrite changes? All .pk4 files have a date before 2018...

Uh, maybe you included only fixed HUD icons (which look proportioned indeed)?
The directories with the textures can be compressed in a zip file renamed zzzpak_hud.pk4
