I've just been playing it at 4k, without any issues.
My quake config was stored at:
L:\SteamLibrary\SteamApps\common\Quake 4\q4base\Quake4Config.cfg
(yours may be on a different drive, just use the "open file location" to get to it.
And I adjusted the following parameters
seta r_customHeight "2160"
seta r_customWidth "3840"
seta r_fullscreen "1"
seta r_displayRefresh "0"
seta r_mode "-1"
after that - no probs at all.

It works. The UI won't tell you that you're running the game at 4k. It will tell you (wrongly) that you're running at 1280x720. You don't have to touch settings through the game UI.

However, I am hoping Quake 4 will eventually get the same remaster as Quake 1 and 2 in the next year or two.
