You and some friends can try TCP/IP connection for DirectPlay, from the Multiplayer menu (you'll need to have the MS DirectPlay API installed).
Depending on your internet settings, you'll need to do port forwarding.

Edit:
There's also GLSMAC, an open-source OpenGL/SDL2 reimplementation of SMAC, that supports multiplayer.

Do you need to port forward if you're using a VPN like Hamachi etc?

If you are the host, you will need to set up port forwarding if your public IP is not the one directly assigned to your machine but to some other one like your router, VPN server, etc (because the port you're using on your PC is not open on your router, and thus the connection to your PC is refused).
You don't need any of it if you are connecting to someone else.
