You think it may have something to do with crashing?
MIA means Missing In Action
If you crash and kill a kerbal they are KIA
If you lose contact with a kerbal like they vanish or go outside the kerbol system or something they are Missing In Action
I thought the respawn timer worked the same for either condition but the originals won't respawn.
Personally if I lose a single kerbal to MIA or KIA I start a new game due to guilt. So I don't know the respawn mechanics in practice.
I build a memorial for them and write a little text in their memory.
Real time gameplay, can't just timewarp and get them back.
The exact respawn timer is in your difficulty settings measured in hours.
