As far as general RCS placement, center of mass or equidistant from and in line with center of mass.
If the craft has a significant difference between wet and dry COM, split the difference.
Also, Caps Lock or "soft control mode" has built in RCS balancing to a degree. As long as you are even remotely reasonable with your placement it should make up for the rest.
As far as the tutorial itself, most of them are pretty buggy; If it doesn't want to complete just skip it.
The trick for this particular tutorial mission is that it wants you to turn the Centre of Mass indicator off again (figured it out by watching a video of some guy that struggled for multiple minutes to figure out 3x ≠ 4x, and what finally enabled the next button was when he disabled the CoM overlay). It doesn't seem to really care about the placement, just that they're present and you've turned the CoM overlay off again, at which point you should be able to click "Next." Hope this helps anyone finding this later, as it is still one of the top results (third for me on DDG with string "ksp advanced construction rcs placement").
