Report a translation problem
I'd suggest moving out of physics range (2.5km) then coming back and re-trying.
That or one of them is on the wrong way around, but it looks fine in the pic as far as I can tell. (The picture is very dark.)
I don't think the docking port is facing the wrong way since I docked both ship A and B to the target multiple times in the past.
Reload game...
Restart machine.
Try F3 and see if any parts are broken... namely the docking port.
(If 2 ships can't dock to that port then it's probably broken)
Try docking to ANY of the OTHER ports available.

load, reload game and restart machine, done all that still nothing
F3 shows no broken part in both vessel A and target

and i tried all available docking port in the target, still no dice
If you are using CKAN to install your mods, then load it up and use the FILE in the upper left and EXPORT INSTALLED MODS to make a *.CKAN file (It just saves a list of installed files)
BACKUP your SAVES directory. AND the SETTINGS.CFG file from the KSP directory.
Uninstall KSP.
MANUALLY delete the KSP directory.
Re-install KSP.
Restore the SAVES directory and the SETTINGS.CFG file to the KSP directory.
Use CKAN to reinstall mods (File - Install from .CKAN)
If you install your mods manually then make sure you are using the latest and greatest of the versions available
Load up the game and see if the problem still exists.
It's just my internet is not really good.
Btw is this a known issue?
It's most likely a mod issue or mod conflict if all of the suggested solutions haven't helped.

I had a problem like this once, a couple of years ago: the docking ship moved into contact range and that "magnet" mode never kicked in, and the ports refused to connect.
It turned out one of the docking ports had somehow ended up in the wrong state. In normal flight, your ship's docking ports are in the "undocked" state. When they're docked to something, your docking ports are in the "docked" state. When you're trying to dock and you get within range, and "magnet" mode kicks in, the docking ports switch to "acquiring", i.e. "hey, I'm attempting to connect".
My ship's docking port was already in the "acquiring" state before I tried to dock. The code has an IF statement somewhere in it, that says "if the docking port is in the UNDOCKED state and the distance to the other docking port is less than one meter, switch this docking port to the ACQUIRING state and activate magnet mode". My ship's docking port was not in the undocked state, so the IF statement's test conditions were never met, so magnet mode couldn't activate.
In order to fix this, I had to open the save file with a text editor, change the state of the glitched docking port, and delete a few lines that were added when the port entered acquisition mode. There's a thread about this problem (with a detailed solution) somewhere on forum.kerbalspaceprogram.com with detailed instructions on exactly what to do. I forget exactly where it is, but I know it's on that site somewhere.
Pretty please with sugar and sprinkles: MAKE A BACKUP COPY of the save file before you mess with it! I have colossally screwed up my game a couple of times while messing with the save files.
