Shattered Realm Reward

I found that there is not a single guide on the Internet that explains the reward for a Shattered Realm.
I accidentally discovered that on the Absolute I was given 3 skill points for passing reality (I don’t remember exactly on 5, 15 and 25 shards, I think).
How can I get all the points? How far do you have to go to get them all? Is it worth it to also collect them on the difficulty below - Elite or there will be another reward?

I'm not a big fan of this mode & dont interested in loot in the end, but for the sake of skill points, I would play.

Afaik, you only get 3 points from SR on ultimate difficulty. So not worth doing the other difficulties unless you want easy looting.
I don't like it either, so I made my own mod to get the skill points elsewhere.
