Face Buttons
A = Action Bar 1
B = Evade
X = Action Bar 2
Y = Action Bar 3

Shoulders
LB = Action Bar 5
RB = Action Bar 4
LT = Hold Position
RT = Action Bar 6

D-Pad
Up = Personal Riftgate
Down = Map Window
Left = Health Potion
Right = Energy Potion

Analog Sticks
Left Click = Weapon Swap
Right Click = Hotbar Swap

I've also dabbled with making "Map Window" a Select/Back long press. Some ARPGs have dabbled with this concept in the past and it's an option if you need to free up another button for an action bar.

Only drawback that I see, dpad buttons aren't recognized with icons on the UI, so health/energy potions won't have an overlay. Would be nice if that was the case.

Also, they just did a minor hotfix (I don't know for what), but if "Evade" Game Actions does not work for you, select Gamepad B as your option, than add Evade as an Extra Command with Regular Press. It will display the B icon again in the UI over Evade.

I just left evade on space. Potions were already on mouse 4 and 5.

In case anyone sees this and wants a gamepad.

For me evade on side mouse button, healing potion on space (you can miss any button in panic situation but you can't miss space, thanks sacred2 for this lesson)

I have my most used skills on asdf as that's the resting hand position for querty typing, with additional skills on qwer for occasionally used skills. Evade goes on g, health on t and energy on v for easy access. Space is reserved for my most used skill to open with like curse of frailty, word of pain, etc.

I have move on spacebar and evade on Q. Changed the codex button to N. Feels good enough to me. I prefer to move around with spacebar cause I don't like to constantly click the mouse both in and out of combat.

123456zxcv for skill
q=for health potion e=for mana potion
f=for pickup items
space still evade

all of this are near to each other, except 6 that would be used for a skilll that has long cooldown.

for kb/m i always use this (in any games)
1234, QWE, ASD > replacing 1234567890 skill button in order
Z > inventory
X > skill
C > codex/character
Tab > Map
R > mana potion
F > health potion
Space > dash
T > Gate

this way i can play with one hand lol, i mean i dont have to look at "I" button for example, just to open inventory, or even "0" to use my last skill, that's too far!!! i literally had to stop and look at my keyboard just to use that?? no thanks

btw many of the main skill i always uses is on Q, W, and E. as well with right click. it's just convenient that way for me. 1-4 usually for passive and ASD is for rarely used/ultimate skill

Each to their own I guess but Ill never understand while people choose to sporadically layout their commands I have a system I pretty much use in every game which allows my left hand to remain at the homekey position and basically never move utilitizing all keys around it.

Mouse side buttons: #1 health potion #2 mana potion #3 evade. These buttons works well, no panicking or frustrated when in heat of battle. I use the spacebar to force standstill, all my skills are on the letters a, s, d, f, g, h, j, k, l. (this setup is so easy and neatly, you dont have to worry about panicking) You can also use the letter q for health and e for mana, if you dont have those extra buttons on your mouse. I only use the letter a s d, those are the 3 main skills that I use the most. (Archon build)
