Depending on how many heroes I stumble upon and whether Eldritch Totem spawns, it takes anywhere from middle part of Tomb of Heretic to entrance part of Court of the Magi for him to spawn for me, if rushing that particular roguelike dungeon.
i feel like Kaisan pretty reliably spawn on Tomb of the Heretic runs or Sunward Spire
you can't just clear small tombs and expect him to spawn everytime, even the other Nemesis don't do that
Dangerous Domains/challenge zones with mutators and skeleton key dungeon increase the spawn multiplier counter
Eldritch Gate stretch is also pretty small/not that densely populated, so even if it has a mutator it's still not a lot it adds total, which could explain the unreliability overall you felt, since it's small+small out of 3 😅
I've no idea what the the potential min or max kill count requirement is, but given the fluctuation it can have even on a ToH run i don't think it's surprising it vary a bit on the route there
