No, they are not reusable. They are one use only. Yes, it was designed that way. If you want more Skeleton keys, you have to craft them. The Steps of Torment and Bastion of Chaos are roguelike dungeons, they are different from the rest of the dungeons in the game. You cannot leave them once entered, and if you die, you cannot go back in unless you have another Skeleton key.
They are meant to be optional end game dungeons. You don't have to do them. They are challenge dungeons.
You were meant to EXIT on like Level 2 or something for the main quest storyline.
The skeleton key bit is purely an optional side quest end-game area that always scales with you.
