All the same rules apply to it as with any DoT or damage mechanic.
what this means is normal flat dmg rules applies combined with regular (true) DoT rules
if you have (flat) Vitality Decay on a regular source, weapon, ring, devotion "not baked into a skill/proc" it's flat dmg as usual and is only applied through weapon attacks and skills with % weapon dmg
if you have a skill with vitality decay dmg, ex Siphon Souls, that skill wont benefit from outside flat Vitality Decay only % Decay boosters (and skill modifiers ofc)
on top of that since Decay is a true DoT it means the DoT rules apply: DoT's from same source doesnt' stack.
This means casting ex Siphon Souls 2x wont yield 2 stacks of Decay, highest hit applies.
Also means, although flat Decay applies via weapon attacks/skills with %WD, you only get 1x weapon dmg source, and highest applies
Ex could be a default white sword "skill" icon attack (the on you start with on hotbar) would be a generic weapon swing at 100% weapon dmg, but Boneharvest could get up to 300% weapon dmg, and would then be your highest %weapon dmg source
